What is cash-based physical therapy—and is it worth it?

Let me be upfront — I’m a cash-based physical therapist.

So yeah, I have a stake in this. But I also spent time working in the traditional insurance-based model, and I’ve seen firsthand why so many people leave physical therapy still dealing with the same issues.

So let’s talk about it honestly.

What Is Cash-Based Physical Therapy?

Cash-based physical therapy is exactly what it sounds like.

You pay directly for your care — no insurance middleman, no deductibles, no authorizations, and no surprise bills weeks later.

At Monarch Performance Physical Therapy in Charlotte, that means:

  • One flat rate

  • One-on-one care

  • Full hour sessions with a Doctor of Physical Therapy

  • No aides, no bouncing between patients

Every session is just you and me — focused on getting you out of pain, moving better, and back to performing at a high level.

What’s the Problem With Insurance-Based PT?

On paper, nothing.

In reality? It’s a different story.

Most insurance-based clinics are forced into a high-volume model. That usually means:

  • 10–15 minutes of actual PT time

  • Being handed off to a tech or aide

  • PT juggling multiple patients at once

Not because they don’t care—but because the system often doesn’t allow them to slow down.

And when you’re dealing with an injury, especially as an athlete or active adult, that just isn’t enough.

How Monarch Is Different

At Monarch, everything is built around quality over quantity.

You’re with me for the entire session. No handoffs. No distractions.

That matters more than people think.

Because real sports physical therapy isn’t passive — it’s constant assessment, adjustment, and coaching in real time.

I’m watching how you move, picking up on small changes, modifying your plan on the spot, and progressing you appropriately — every single session.

That level of attention is what allows us to actually progress you—not just keep you busy.

We Treat the Root Cause — Not Just the Diagnosis

Insurance ties care to a diagnosis code.

Your body doesn’t work that way.

If your knee pain is coming from your hips, your ankle mobility, or your running mechanics — we’re addressing all of it.

If you started care for your shoulder but tweaked your back over the weekend, we adjust care and focus on what matters most to YOU that day.

That also means we can be proactive by:

  • Catching issues early

  • Addressing movement limitations

  • Prevent injuries before they happen

This is where injury prevention, performance therapy, and return-to-sport training all come together.

Fewer Visits. Better Results.

This isn’t about rushing the process.

It’s about doing it right.

When you have full one-on-one attention and a clear plan, you don’t need endless visits.

Most people get more out of 6–8 focused sessions here than they did in 12–16 sessions in a traditional clinic.

That’s the difference between just going through exercises… and actually progressing.

What About the Cost?

In many cases, patients end up paying similar out-of-pocket costs—but for less time, less attention, and slower progress.

What most people don’t realize is that if you have a deductible, you’re often paying close to full price anyway—just without the level of care you expect—and sometimes with a bill showing up weeks after your visits are already done.

At Monarch, it’s simple:

  • No hidden fees

  • No surprise bills weeks later

  • No insurance limitations

Just clear, upfront pricing—and a higher level of care that gets you better in fewer visits.

Who This Is Perfect For

This model works really well if:

  • You’ve already done PT but didn’t fully get better

  • You were discharged because of insurance, not because you were 100%

  • You keep dealing with recurring injuries

  • You want to understand why you got hurt in the first place

  • You’re an athlete or active adult who wants to return to sport stronger than before

A lot of my patients come in frustrated because they’ve been stuck in that cycle.

Pain → rest → feel better → pain comes back

That’s what happens when the root cause never gets addressed.

We fix that.

What Now?
