The real problem

Rest is not a plan.

Your back locks up after deadlifts. It aches on every run. Or an MRI report gave you a scary name for it, and now you avoid every lift that might make it worse.

That avoidance is often what keeps the pain around. Movement and progressive loading are two of the best supported treatments for low back pain. The spine is one of the most robust structures in the body, and it adapts to load the same way a muscle does.

Imaging shows what your back looks like. Movement shows what it does.

The Monarch Method

Every plan runs through the same four phases.

Our simple but effective formula for success.

  1. 01

    Assess

A full movement exam under load, not only a look at the MRI. Here we find where the breakdown happens.

  2. 02

    Restore

Calm the back down so you can move again. Here we can use manual therapy and dry needling.

  3. 03

    Rebuild

Progressive loading of the hips, the trunk, and the back, so your back handles more than before.

  4. 04

    Perform

Return to deadlifts, squats, runs, and workouts, with a plan to keep the pain from coming back.

What we look at

Find the source, then build the plan.

When an athlete comes in with low back pain, we do not lead with the MRI. We look at how you move, and where it breaks down under load.

  • A hip mobility limit that puts extra arch in your low back on a deadlift
  • A core stability gap that shows up when you are tired
  • Where the pain comes from: a disc, a facet joint, the SI joint, or a muscle
  • How your training load changed before the pain started

Should I stop lifting or running with back pain?

Usually not completely. Avoiding all load is often what keeps the pain around. We change what you do, not whether you train, and we build your back to handle more load over time.

Do I need an MRI before physical therapy?

Not to start. Many people with no pain at all have disc changes on an MRI, so the image alone rarely explains the pain. We look at how you move under load. If anything in your exam needs a doctor or imaging, we tell you and refer you.

Do I need a referral?

No. North Carolina allows direct access to physical therapy. You can book your first visit today.

What happens at my first visit?

A full 60 minutes, one-on-one. We go through your injury and training history, then a full movement, mobility, and strength exam. You leave with a plan and exercises to start right away.

How much is a session?

Each 60-minute session is $175. You can pay with HSA or FSA.

Do you take my insurance?

Monarch is an out-of-network, direct-pay practice, so every session is a full hour built around your goals, not around an insurance rule. You can also send your receipt to your insurance for possible reimbursement.
